AISTAR Standard Forms

📋 Instructions: Click on any "Download" link to access the form. Forms are in DOCX format (editable). Complete all relevant sections, sign where required, and file with the AISTAR Registry through the ACDS or by email to info@aluochier.co.ke. Filing fees are KES 2,000 per filing unless indigent status applies (Form B35).

📌 Note on Form B44A (Optional Signature Page): Form B44 (Settlement Application) references an optional B44A signature page. This is not a separate numbered form but an annexure that may be created as needed when the number of primary parties exceeds the space on Form B44. Any sheet listing party names with signature lines and dates is sufficient.

📁 File Format: Forms are provided in DOCX (Microsoft Word) format. If you do not have Microsoft Word, you may open these files using Google Docs, LibreOffice, or other free word processing software that supports the DOCX format.
